DIRECTOR, DISEASE MANAGEMENT

Scope of the Job:

This new position, located at Corporate Headquarters in Houston, Texas, reports to the Vice President / General Manager of Medical Oncology. Your primary responsibility will be to direct and oversee the objectives and goals of the Disease Management Program. You will direct consistent, high quality telephonic care to the members by effectively leading and supporting the Disease Managers and staff.

Your responsibilities will include:

Lead and develop Disease Managers (DMs).

Direct and participate in hiring, orientation, staffing and development of DMs, ensuring high quality care through appropriate utilization of resources.

Ensure consistent development and retention of DMs and clinicians.

Develop, prepare and analyze reports summarizing overall operational and financial performance of team.

Coordinate with DMs to ensure appropriate performance, manage clinical and financial outcome results and ensure level of consistency across DMs.

Collaborate with other internal departments in the operation of the Disease Management unit.

Constantly focus on understanding the call center process and results and using the knowledge to improve the delivery of program and quality performance.

Support and execute the Quality Monitoring Program and assure accreditation and state surveys, as required.

Serve as clinical liaison for account managers and work to facilitate development of provider relationships through interfaces with facility representatives and network physicians.

Manage the partnerships with external vendors from an operations perspective that provide elements of the programs (lab, pharmacy, radiology, home health vendors, etc).

Contribute to overall strategic development and sales efforts around market penetration.

Responsible for the department's operations, performance, & budget.

As necessary, serve as external customer liaison for both Payer Customers and Physician Practices.

Candidate Requirements:

We seek a healthcare professional with 8-10 years experience including 3 or more years with Disease Management Call Center, Nursing or pharmacy experience.

Candidates should possess clinical care experience and understanding of call center operations. A business background with 5 – 8 years of progressive clinical management experience is preferred.

For the best fit you should be a results-oriented team player with strong interpersonal and communications skills, capable of working collaboratively with colleagues.

Candidate Education and Training:

We seek a professional with a Master's Degree in Nursing/Healthcare Administration or Business Administration.

Current active, unrestricted Texas RN license, strongly preferred.

Certified Case Manager or Case Management Administrator Certified preferred.

Certified Profession of Healthcare Quality (CPHQ) preferred.

Compensation:

A compensation package will be designed to attract outstanding talent and will include a base salary, performance bonus, and appropriate stock incentives.
